Effective January 2010, the New Jersey Legislature amended N.J.S.A. 18A:40-12, and added provisions to enhance the medical management of students with diabetes in the school. This amendment requires additional information to be provided to the school to care for your child.
The attached documents are required to be submitted to the school each year. Please have your physician/medical provider complete and sign the Diabetes Medical Management Plan for your child, and return it to the School Nurse as soon as possible, or at least one week before the first day of school. Please also sign the plan authorizing the school to carry out the tasks outlined by your child’s plan.
